These words still guide every employee, every day. Our guiding mission is to help kids and their families' transition from hospital to home. That means we practice world-class pediatric medicine to treat the symptoms but we also train parents and caregivers to provide a high-level of care at home so patients continue to progress after discharge. "Care Beyond the Bedside" means that our kids receive their therapies and rehab away from their rooms and out of their beds as much as 70% of the day. The rest of the time, they are playing just being kids in our state of the art facility designed to inspire children. Job Summary : The Skin & Wound Care Nurse supports the development and implementation of pressure injury prevention and skin care initiatives that align with the overall Skin and Wound Care Program at Ranken Jordan. Collaborates closely with the WOCN Nurse Practitioner, the Medical Team, and Clinical Services to meet the skin and wound care needs of Ranken Jordan's inpatients. Blood Borne Pathogen Exposure: Yes DUTIES AND RESPONSIBILITIES SKIN AND WOUND CARE PROGRAM IMPLEMENTATION (80%) Assessment: Performs and documents skin assessments and wound care assessments when they develop or when patients are admitted. Demonstrates skills of a knowledgeable clinician and able to recognize differences between normal vs abnormal physiological and emotional behavior and to effectively communicate any deviations to the medical staff. Continually alert to changes in patient's condition, attention to details, uses initiative and sound judgment, interacts with tact and kindness toward patients and caregivers and applies nursing principles and technique. Performs duties with a high degree of accuracy when recognizing symptoms, following orders, and initiating and documenting nursing care measures. Communication : Collaborates with all departments to maintain a teamwork approach. Articulates the role and responsibilities of the Skin and Wound Care Nurse to team members. Uses clear and concise documentation of information in the medical record. Always maintains strict confidentiality regarding patients and other confidential information. Provides nursing staff, patients, and caregivers information and education regarding skin and wound care needs and condition changes. Consistently creates and contributes to a positive work environment through attitude, ability to work with others, and the ability to communicate clearly and concisely. Planning: Works closely with the WOCN Nurse Practitioner and Medical Team to review and evaluate each patient's skin and wound plan of care and perform wound care treatments. Uses evidence-based knowledge and research findings to guide practice and develop strategies/interventions to manage care to achieve appropriate goals and outcomes for patients with acute or chronic wounds. Participates in weekly Skin Rounds to prevent, detect, and treat skin injuries. Stocks patient wound care supplies on a routine basis. Coordinates and schedules patient bathing. May participate in multidisciplinary patient care conferences. Professionalism: Functions within guidelines set forth by the State Nursing Practice Act, nursing department and hospital. Represents nursing at Ranken Jordan in a professional manner through communication and appearance. Quality & Safety : Strives to deliver patient care in the safest manner possible. Follows Ranken Jordan Policy regarding hand hygiene and all infection prevention measures. Provides skin and wound care correctly following medical staff orders. Evaluates processes, policies, procedures, and protocols/guidelines for care of patients with acute or chronic wounds; and recommends revisions when warranted. Conducts skin and wound care-related audits to monitor the effectiveness of interventions and completeness/accuracy of documentation. Participates in appropriate clinical quality workgroups and/or initiatives when applicable. Active membership in Nursing Shared Leadership is encouraged. Education : Develops and implements evidence-based education and educational materials for patients, staff, and other healthcare providers. Teaches patient, family, and caregivers how to successfully meet patient's skin and wound care needs, realizing any cultural diversities or educational barriers. Engages patients with acute or chronic wounds in self-care to maximize independence and achieve goals for quality of life. Collaborates with Nursing Educators to support on-going staff education in skin and wound care topics. PROFESSIONAL ACTIVITITIES (15%) Keeps current on issues, practice and new trends related to health care, specifically pediatric care, pressure injury prevention, and wound care. Attends and participates in continuing education programs, seminars, etc., designed to maintain professional competence. ALL OTHER DUTIES AS ASSIGNED (5%) by the Nurse Manager, Director of Nursing, and Chief Nursing Officer. QUALIFICATIONS REQUIRED: Background Requisites: A minimum of 2 years of relevant hospital experience with exposure to skin and wound care best practices. Must have a current license to practice as a Registered Nurse in the state of Missouri and hold a current Basic Life Support (BLS) certification from the American Heart Association. Current Pediatric Advanced Life Support (PALS) certification or completion within 12 months. Certified Wound, Ostomy, and Continence Nurse (WOCN). Baccalaureate degree required. PHYSICAL DEMANDS : Include stooping, kneeling, crouching, reaching, grasping, pulling, pushing, standing, carrying and lifting of light loads (up to 35 lb.).
