ACT Team Case Manager

Description:
The Act Team Case Manager is responsible for providing high quality care in the community/outpatient settings to ACT team consumers. The ACT Team Case Manager is expected to function well as a member of the multidisciplinary Clinical Team. The Act Team Case Manager is responsible for utilizing professional skills in assessment, treatment planning, case management, crisis intervention, individual and group supportive counseling, and the documentation of services provided in direct care of ACT team consumers. The Act Team Case Manager is primarily responsible for targeted case management services, service coordination, and supporting activities of daily living and transportation. All services provided are to promote client independence and empower clients to manage their symptoms to insure an acceptable level of daily functioning. Qualifications: Bachelors Degree in a behavioral health related field. One year experience in behavioral health with substance abuse and/or vocational experience preferred. Must have a valid driver's license and a clean driving record. Twenty-four hour accountability and responsibility for case management services is required as needed to ensure the highest quality of consumer care and safety at all times
