Fedelta Home Care is hiring a Per Diem/PRN Registered Nurse to service patients located in and around the Portland, Oregon area. Fedelta Home Care is a fast-growing in-home care company that seeks to provide only the highest caliber of care for our patients. Essential Job Responsibilities:  Conduct initial in-home prospective patient evaluations and/or assessments.  Establish patient's individualized service plan and client record baseline.  Collaborate with agency staff and contract services to implement the patient's service plan.  Supervise the implementation of the service plan and the quality of care delivered to the patient.  Train new caregivers on caregiver duties and evaluate through return demonstration competency in medication administration and personal care tasks.  Provide in home supervision of caregivers and provide review of medication regimen at least every 90 days for patient receiving aide services; ensure proper documentation within the client record.  Review completed MAR each month for completion and errors.  Create new monthly MAR based on doctor's current medication orders.  Delegate nursing services as outlined in policy including assessment of client and caregiver for appropriateness.  Collaborate with Pegasus team members to facilitate timely office processing of paperwork and personnel information.  Actively participate in agency quality improvement activities.  Flexibility with schedule to address urgent needs.  Travel throughout Portland metro area including outlying distances such as Forest Grove and Sandy is required.
