The Job You've Been Waiting For! AuthoraCare Collective is currently seeking an Care Coordinator LPN for our Palliative Care and Chronic Disease Management position. This is a full-time position responsible for supporting Burlington and Greensboro campuses. Job Summary: Provides high quality nursing services under the direction of a Registered Nurse/nurse practitioner/physician consistent with the agency's philosophy, policies and standards of care. Supports and works closely with the the palliative care and chronic disease management team by evaluating/interpreting clinical data, maintaining workflows, acting as an internal/external liaison, providing clinical expertise via phone calls, care coordination and support as well as telephonic/in person visits as needed. This position will also be an integral part of data analysis and workflow integration to efficiently serve the patient population. Data analysis- Constantly able to examine and evaluate data through gathering, collating or classifying information interpreting and delivering results. Constantly able to coordinate time, place and sequence of operations or action to be taken. Constantly able to prioritize multiple responsibilities and/or accomplish them simultaneously. Utilization Management/Risk assessment. Excellent nursing skills, problem solving and critical thinking ability and effective interpersonal and communications skills and computer competency necessary. Coordination of care. Ability to collect information and then disseminate/act as appropriat e. Education and Experience : Must be a graduate of an accredited LPN program. Must have a current North Carolina LPN license. Two year's LPN experience necessary, preferably one year in an acute care, hospice, LTC or home health setting. AT least 2 years of previous and proven experience in analytics, data interpretation, workflow development and process alignment required Utilization Review experience preferred Vaccination against COVID-19 is mandatory at our organization unless you are approved for an accommodation due to a religious objection or ADA covered medical condition. The organization will also review medical recommendations for a delay in vaccination or for other contraindications to vaccination.
