Hands-on training with opportunities for advancement Position Description Provides direct personal patient care in a patient's home according to a patient's specific plan of care prepared by the RN Case Manager. Promotes patient comfort and dignity. Major Responsibilities Provides aide services to agency patients as identified by the RN Case Manager in the plan of care per HHH policies, procedures and aide skill level. Prepares and maintains accurate and timely patient notes, documenting interventions, patient pain score, outcomes and patient/representative response per HHH policy. Participates in care coordination and supporting the implementation of the care plan within the scope of practice. Provides total or assisted personal hygiene, per plan of care and assists with activities of daily living (ADL) and instrumental activities of daily living (IADL) allowing patient independence to tolerance. Provides assistance in ambulation, exercises, transfers and medication compliance by patient or caregiver per therapist or RN plan of care. Assists in maintaining a clean, safe patient environment (IADLs) necessary to the patient's healthcare needs, including assisting in feeding patient, as needed and documenting intake per plan of care instructions. Measures and records oral, rectal, auxiliary temperatures, pulse, respiration and blood pressure as requested by a nurse on the aide plan of care. Provides social interaction activities as directed by the plan of care. Reports patient condition changes and pain score to the RN Case Manager. Practices safety and standard precautions, infection control and uses appropriate protective equipment to protect patients and self. Education and experience required : High school diploma or GED required WA State Nurse Assistant-Certified (NA-C) certification required BLS required, active prior to start Current valid state driver's license and proof of auto insurance required Schedule 0.8 FTE; 32 hours/week; Day shift Application Instructions To apply for this position and future positions, please visit our careers website at https://jeffersonhealthcare.org/healthcare-careers/ This position will remain open until filled.
