POSITION SUMMARY The Advanced Practice Registered Nurse is responsible for working in collaboration with the patient, primary care physician, medical director, clinical nursing leadership and other palliative care team members in an active practice to deliver hospice and palliative care and related health care services to patients and families. Primary Responsibilities Available as a consultant to patients, families and staff regarding end of life issues and to collaborate on symptom management, improve the patient and family experience of care, and improve the efficiency and effectiveness of resource use. Provides direct patient care (or assists others to provide such care) to dying patients and their families in collaboration with the patient's primary care physician, the hospice physician, hospice nursing staff and volunteers, and other members of the care team including Social Work and Chaplain Services. Care is directed at meeting the physical, psychological, social and spiritual needs of the patient and family. Prescribes or recommends drugs or other forms of treatment. Records physical findings, and formulates plan and prognosis, based on patient's condition. Consults with the Medical Director and palliative care physician(s) as needed: consults primary physician of services provided and collaborates with other physicians to prepare comprehensive patient care plan. Monitors and maintains progress of patients and reports progress to Medical Director. Has the ability to work collaboratively with hospice employees and volunteers as part of an IDG: Participates in Interdisciplinary Group Meetings Provides coverage of on-call during the extended hours and weekends as needed Provides training and continuing education to staff in collaboration with the education department Assists in development of clinical practice guideline/standards/policies/procedures in support of quality palliative care Identifies needs within different stakeholder organizations and recommends the development of systems within Hospice Hawaii to meet those needs Face to Face Visits Completes required face-to-face visits as assigned Providing clinical assessment results to the hospice medical director after making face-to-face visits with patients, as assigned Participating in the recertification process and collaborating with the Interdisciplinary Team to promote coordination of patient care Maintaining accurate, up-to-date records Other related duties as assigned by the Medical Director. Knowledge and Skills Requirements Demonstrates sound decision-making in the care of patients and families though use of the nursing process Ability to work independently; demonstrate sound judgment in planning and providing patient care. Skilled in providing holistic nursing care to patients. Ability to identify psychosocial needs of caregivers and provide basic care to meet those needs. Ability to establish and maintain effective working relationships with staff, clients, family members, the public, and representatives of other agencies and organizations. Ability to communicate effectively with all levels of individuals. Experience and Educational Requirements State of Hawaii Registered Nurse license - current and in good standing State of Hawaii Advanced Practice Nurse (APRN) - current and in good standing Current DEA / State NED certification and full prescriptive authority Eligible to obtain NPI/ Medicare enrollment in order to allow Hospice Hawaii to bill A minimum of two years experience of direct patient care experience required ELNEC training and experience with hospice and palliative care services preferred HPNA certification preferred Excellent interpersonal and communication skills Valid driver's license and use of a reliable, registered and insured automobile Employment Requirements: All offers are contingent on a candidate's successful completion of the following requirements: A legally compliant background check Drug Screen & Physical Exam Current TB clearance CPR certification Use of personal motor vehicle which meets valid registration and insurance requirements. Must possess a valid Drivers' License, current auto insurance, and reliable transportation. Must have acceptable driver's abstract. COVID Vaccination Hours: Generally, Monday through Friday, 8:00 am through 4:30 pm. Requires intermittent On-Call supervisory coverage Why Navian Hawaii?
