FITT Therapist - Full-time Opportunity - Lee County (Sign-on Bonus) Position: FITT Therapist, $2,500 Sign-on Bonus Location: Lee County, FL Charlotte Behavioral Health Care's (CBHC) is our area's largest not-for-profit provider of community-based behavioral health care, offering a full range of mental health and substance use services in Port Charlotte, Punta Gorda, Englewood, North Port, Sarasota, and DeSoto and Lee Counties. Last year, we served over 11,000 individuals and families. CBHC's mission is to partner with our patients to instill hope, inspire growth, and embrace life. CBHC strives to provide compassionate care to individuals with mental health or substance use disorders through a variety of programs. Our goal is optimal behavioral health and improved quality of life for all children, adolescents, adults, and families. Program Summary The Family Intensive Treatment Team (FITT) model is designed to provide intensive services to families in the child welfare system with parental substance use. Upon successful completion, the family should have the skills and natural support system needed to maintain improvements made during services. The goals of the FIT Team model are to: Provide early identification of at-risk families and immediate access to intensive substance use and co-occurring mental health treatment services for parent(s)/guardian(s) in the child welfare system with early engagement strategies, such as at case initiation or case transfer, when a child in the family has been determined to be "unsafe"; Establish a team-based approach, including Clinicians, Case Managers and Recovery Peer Support Specialists, to planning and service delivery in coordination with Community-Based Care Lead Agencies, Child Welfare Professionals, Managing Entities and other providers of services; Integrate evidence-based treatment for substance use disorders, parenting interventions, and therapeutic treatment for all family members into one comprehensive treatment approach. This comprehensive approach includes coordinating clinical children's services, which are provided outside of the FIT Team funding; Identify family-driven pathways to recovery and promote sustained recovery through cultural and gender sensitive treatment and involvement in recovery-oriented services and supports; Promote increased engagement and retention in treatment; Provide 24/7 access for crisis management; Facilitate concurrent planning between child welfare case planning and treatment plan goals, to integrate the family's strengths and needs with their dependency case plan; Advocate for parent(s)/guardian(s) and assist in navigating the child welfare process; Promote treatment completion and continued care through linkage to ongoing support services and natural supports; and In collaboration with Community-Based Care Lead Agencies and Child Welfare Case Management Organizations: Promote safety of children in the child welfare system whose parent(s)/guardian(s) have a substance use disorder; Develop a safe, nurturing and stable living situation for these children as rapidly and responsibly as possible; Provide information to inform the safety plan, ongoing Family Functioning Assessments (FFA), and any other relevant status updates; Reduce the number of out-of-home placements when safe to do so; and Reduce rates of re-entry into the child welfare system. Position Summary Under the supervision of the Program Manager, this position strives to make a meaningful difference in the lives of families impacted by co-occurring mental health and substance use disorders. Utilizing a team approach, this position works together with a Case Manager and Certified Recovery Peer Support Specialist to provide therapy, advocacy, and relapse prevention skills which aim to support parents in safely caring for their children. Through applying evidence-based interventions on a weekly basis, the FITT Therapist is able to help parents make lasting therapeutic changes, thus positively affecting the lives of children across Lee County. Treatment provided by FITT is offered in the community including local settings such as parks, libraries, offices and the patient's home. FITT works closely with Child Welfare Case Managers to assist parents in completing case plan tasks, monitoring safety plans, and increasing protective factors. This includes regular communication and case reviews to promote progress, child safety, and collaboration. Child Welfare and FITT collaboration efforts strive to develop a safe, nurturing and stable living situation for children as rapidly and responsibly as possible, to reduce the number of out of home placements when safe to do so, and reduce rates of re-entry into the child welfare system. Responsibilities: Provides immediate access to substance use and co-occurring mental health treatment in the community with a caseload of no more than 15 patients. Supports patients in exploring multiple pathways to recovery including harm reduction, medication assisted treatment (MAT), and abstinence. Completes biopsychosocial, ASAM, treatment plan, discharge summary, clinical notes and other forms of documentation or assessment within designated timeframes. Consistently achieves established productivity thresholds. Completes monthly progress reports on behalf of patient caseload. Provides appropriate individual, family, and group therapy. Willingness to work flexible evening hours. Able to work independently and manage a flexible schedule. Rotates weekend on-call crisis management with other team members (financial incentive provided). Completes monthly progress reports assessing Caregiver Protective Capacities. Participate in weekly Treatment Team Meetings and individual supervision. Participate in ongoing trainings and staff meetings. Learn facilitation in Matrix Model and Seeking Safety. Receive free clinical supervision if not yet licensed. (Available after 90 days of successful employment) Become certified in facilitating parenting classes Minimum Requirements: A Master's or Doctoral degree in behavioral health sciences, such as mental health counseling, social work, art therapy, psychology, or marriage and family therapy. A minimum of two (2) years of experience working with adults with substance use disorders. Must maintain high standards of ethical and professional conduct, while adhering to agency policies and procedures Ability to pass Level II Background clearance, drug test (CBHC is a drug free workplace) Valid FL driver's license, insurance, and safe driving record Dependable transportation (registered, safe operating conditions, etc.) Preferred: Experience working in Child Welfare Why Charlotte Behavioral Health Care?
