CATHOLIC HOUSING AND COMMUNITY SERVICES Social Worker - Social Service Care Manager Catholic Housing and Community Services is currently seeking a full-time Social Service Care Manager (40 hours/week) to be based in Manayunk and Norristown. Primary responsibilities of this position include: • Conduct home visits and follow-up telephone calls to provide assessment, care planning, and supportive services as needed to the senior population, carrying a caseload of approximately 65 clients in the geographic boundaries of two parishes. • Perform assessments to determine strengths and barriers to healthy aging. • Develop a care plan with the client to set goals and identify corresponding interventions and resources. • Connect clients to appropriate services that enhance their quality of life. • Keep resident files up to date in the social service database including case notes, diagnoses, key contacts, assessments, and care plans. • Provide individual and group counseling and supportive services as needed by the clients and parish communities. • Perform outreach and advertising to engage potential clients in the program. • Assist with responding to external phone calls for information and resources. Position Requirements: • Master's degree from an accredited university in Social Work, Gerontology or related field preferred. • Experience working with seniors preferred. • Knowledge of the local community, resources, federal and state benefits programs and their eligibility requirements. • Must have a current driver's license, vehicle insurance, reliable transportation and be able to meet the Authorized Driver criteria as required by the Archdiocese Office for Insurance Services.
