SUMMARY Provide comprehensive, home-based therapy to youth and families receiving home-based treatment services. Serving families with one or more children (up to age 18), who are diagnosed as having a serious emotional disturbance (SED) and families having multiple service needs. Services are youth and family-centered and build on the youth and family strengths to increase personal and community supports, promote healthy family functioning, support, and preserve families, and reduce the need for out-of-home placement. Services are provided in-home/community through flexible scheduling. Per Diem positions are positions based on job duties assigned such as providing individual therapy. In this position you will be compensated for direct therapy services and administrative duties required under this treatment model. Direct therapy service is any time spent with the individual served that is considered billable under HPCT code definitions. Administrative time is any time spent for travel, meetings, supervision, documentation, and trainings ESSENTIAL DUTIES AND RESPONSIBILITIES: Responsibilities associated with this job will change from time to time in accordance with the agency's program and business needs. Offer a variety of clinical interventions including best practice and evidence-based models. Development of Individualized Safety/Crisis Plans with Families. Provide services during after-school and evening hours as well as possible weekend appointments when requested. Provide accurate monitoring and documentation of all case related activities to fulfill program obligations and requirements. Pursue broadened professional development to enhance level of competency and meet training requirements of contract. This position will be knowledgeable about and actively support 1) culturally competent, recovery-based practices; 2) person-centered planning as a shared decision-making process with the individual, who defines his/her own life goals and is assisted in developing a unique path toward those goals; and 3) a trauma informed culture to aid individuals in their recovery process. Adhere to Agency and program philosophies, policies, and procedures to assure consistent and ethical professional standards and required service outcomes. EDUCATION and/or EXPERIENCE: Minimum MSW/MA in a mental health-related field from an accredited school Individual with specialized training and one year of experience in the examination, evaluation and treatment of minors and their families Must be trained in CAFAS or ability to be trained as a reliable rater of CAFAS Lived experience with behavioral health issues is desired Education, training, and/or knowledge of the impact of trauma on individuals, children, and families, is desired. Trained or willingness to be trained in First Aid, CPR, and AED LANGUAGE SKILLS: Ability to read and interpret documents such as safety rules and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups, customers, or employees of the organization. Ability to understand and motivate people and an awareness of importance of cultural differences within service populations. Operate as a team player with excellent organizational and communication skills. REASONING ABILITY: Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations. Ability to solve problems within a broadened area of knowledge involving somewhat differing situations and specialized standards. CERTIFICATES, LICENSES, REGISTRATIONS: Required: Valid Licensure as LLMSW, LMSW, or LLPC, LPC in the State of Michigan. Full licensure preferred. PHYSICAL DEMANDS: The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. Ability to regularly provide out-of-office services in unstructured, unpredictable situations. Trained or ability to be trained in First Aid and CPR. While performing the duties of this job, the employee is regularly required to stand; use hands to finger, handle, or feel; and talk or hear. The employee frequently is required to walk and reach with hands and arms. The employee is occasionally required to sit; climb or balance; and stoop, kneel, crouch, or crawl. The employee must occasionally lift and/or move up to 25 pounds. Specific vision abilities required by this job include distance vision, peripheral vision, and depth perception. While performing the duties of this job, the employee must be able to travel to all areas within the service area. Must be able to transport clients in own vehicle. Ability to drive own vehicle and maintain a valid, unrestricted driver's license and vehicle insurance that meets agency requirements is required for this role. Ability to drive during the day or evening, and in varied weather conditions, is required for this role. Home visits are often made during evening hours as required to meet family need. A small percentage of home visits may need to be made on weekends or holidays.
