PURPOSE OF POSITION This position supports and assists families as they identify and meet their own goals through a family-centered case management model for developing and carrying out the program Family Partnership Agreements, including responsibility for child files, enrollment/applications, etc. This position also assists with completing the child health requirements and other required screenings and assessments. This position supports the larger social services system coordinated by Early Head Start, thereby supporting the program's goals. The position may involve reassigning caseloads and/or center locations as necessary for program operations. Role and Responsibilities The duties of the position are described in the major functional areas listed below. Additional duties may be assigned. EXAMPLES OF SPECIFIC DUTIES-- ESSENTIAL JOB FUNCTIONS Program Development, Planning, and Implementation · Coordinates with program staff to reach an integrated approach with family partnership goals. · Participates in team management functions of planning, implementing, and evaluating Parent, Family and Community Engagement Outcomes (PFCE). · Participates in training activities to enhance job related skills. · Recruits prospective families and assists with the enrollment process. · Attending home visits to facilitate family plans, assisting families in achieving identified goals, and follow-up on referrals. Acts as a case manager and works cooperatively with local agencies to assist families in linking up with services in the local community. · Assists with completing the child health screenings, other required screenings, and is familiar with referral requirements. · Conducts and documents follow-ups on all referrals. · While respecting family confidentiality, share information with teachers to ensure coordinated services that meet the needs of individual children and families. · Provides health, safety, and family literacy activities to parents, as appropriate. · Coordinates with center staff to promote parent involvement in the program as a classroom visitor, volunteer, employee, and/or participant in family events. · Promotes effective community support for families by coordination and advocacy for services with community agencies. · Develops a working knowledge of community resources. At least annually, visits with major agencies for service delivery updates. · Refers families to appropriate agencies for emergency or crisis assistance. · Participates in chart audits and child profile staffing. · Coordinates or refers families to opportunities for continuing education, employment training, and other employment services through community networking. · Assists other program staff to offer (either directly or through referral to local entities) to parents of participating children the following: · training in basic child development. · assistance in developing communication skills. · opportunities to share experiences with other parents. · In-home visitation as needed; and · any other activity designed to help parents become full partners in their children's education. · Assist family with completing the needs assessment. · Performs community outreach to encourage individuals previously unaffiliated with Early Head Start program to participate in its Early Head Start program as volunteers. · Arranges for education and other appropriate intervention, including opportunities for families to participate in counseling or to receive information on mental health, substance abuse, child abuse and neglect, and domestic violence. · Attending parent/teacher conferences and home visits, as requested. · Facilitates and supports monthly Parent Committee meetings at the Center. · Assists teaching staff with Family Nights, socialization, and other parent activities. · Assists families in making the transition into and out of the Early Head Start · Ensure an appropriate written Transition Plan for all children six months before their third birthday; provide parents with training and information. Establish communication to facilitate and coordinate the transition. · Provides to parents of limited English proficient children outreach and information in a language that the parents can understand. · Tracks attendance and works as a partner with parents to encourage regular attendance. · PERFORMS OTHER JOB DUTIES AS ASSIGNED Recordkeeping · Documents efforts to assist families in identifying their strengths, needs, and sufficiency goals. Performs data entry functions as necessary to keep files up-to-date. Uses GoEngage software. · Tracks family partnership efforts and Parent and Family Engagement Outcomes. Documents efforts to support parents in establishing strong community ties, such as finding a "medical home." · Records, monitors, and follows up on referrals for support services for reporting requirements. · Submits reports as needed and requested, such as monthly summary tracking reports. Follows up with families to determine whether the kind, quality, and timeliness of the services received through referral met the family's expectations and circumstances. · Maintains and provides data for the annual Program Information Report (PIR). EXAMPLES OF GENERAL JOB DUTIES-- ESSENTIAL JOB FUNCTIONS · Understands, generates, and documents in-kind and other allowable costs applied toward the non-federal share requirement. · Attending all workshops, training courses and meetings as deemed necessary by the immediate supervisor. · Attends all required staff and parent meetings and activities. · Responsible for understanding Head Start Performance Standards and local childcare licensing regulations. · Expected to be knowledgeable of the regulations associated with preventing occupational disease and injury, including the exercise of universal precautions and the prevention of contamination. · Maintains strict confidentiality concerning Early Head Start children, families, and staff, following established policies and procedures. · Become thoroughly familiar with Children's Coalition for Northeast Louisiana Head Start Employee Handbook: Human Resources Policies and Procedures, Children's Coalition for Northeast Louisiana Head Start Operations Manual, and all other Children's Coalition for Northeast Louisiana HS handbooks, manuals, and resources adhere to them. · Implements the plan and system of continuous monitoring and evaluation of activities within the program to assure that the family's needs are being met (family outcomes). · Establishes relationships with community partners and identifies resources that can be made available to families. Initiates and tracks community partnership agreements. Establishes and monitors contracts for services (e.g., ABE/GED courses, food banks, clothing banks, child support enforcement, etc.)Performs Other Duties as Assigned by Supervisor Education and Experience · The minimum requirement is an associate degree or higher in Social Work, Human Services, or a related human services field. · Requirements for employment with Children's Coalition for Northeast Louisiana Head Start may differ from those required by the former grantee or a replacement grantee. If applicable federal, state, or local Head Start or childcare regulations, now or as amended in the future, contain additional requirements or qualifications for this position that are not explicitly stated in this Job Description, the requirements, and qualifications will apply, even if not explicitly outlined in this Job Description. · Bilingual preferred (English/Spanish or English and other languages present in the local area). Acceptable tuberculosis screening results, a clear criminal records check (including a child abuse registry check if required by state regulations), and an initial health exam are required post-job offer and before employment. To drive agency vehicles (if applicable), one must have a valid driver's license, be insurable by the Children's Coalition for Northeast Louisiana Head Start insurance carrier, and have a Motor Vehicle Record acceptable to Children's Coalition for Northeast Louisiana Head Start. Current and former Early Head Start parents will receive preference for employment vacancies for which they are qualified. · Must have reliable transportation and, if/when using own vehicle, must have minimum automobile coverage required by state of employment. Physical Demands and Working Conditions · May be required to lift to 40 pounds. · Will be required to perform physical activities to engage with toddlers/Infant including bending, squatting, getting down and up of the floor. · Travel by car, bus, airplane, or train may also be required to attend conferences, meetings, and other duties carried out at distant locations in and out of state, and in some cases, overnight travel may be required. · Some exposure to communicable diseases. · The noise level in the work environment may be moderate to loud. · Work is typically performed in a classroom, kitchen, or bus setting, and some office work will be required. · Evenings and weekend work may occasionally be required for home visits, parent-teacher conferences, Parent Committee meetings, community, and social events.
