Village Care Center in Erlanger, Kentucky is hiring a FT, MDS Coordinator to join our team. Title: MDS Coordinator Department: Administration Status: FT Shift: Days What will I do as the MDS Coordinator: Receive direction from the Nursing Home Administrator on overall QAPI objectives for the organization. Be knowledgeable in data collection, data analysis methodology and performance improvement methods needed to support and lead performance improvement projects. Identify opportunities for improvement through analysis of data, observation of operations and consultation with leadership and staff. Collaborate with the quality committee and senior leaders to prioritize and develop QAPI efforts. Lead performance improvement projects and provide education and coaching in order to build needed skills in others to lead PIPs. Participate in multidisciplinary QAPI activities. Requirements for MDS Coordinator: RN License required in the state of KY Three years experience as an LPN Minimum 1 years' experience with MDA completion Minimum 1 year of supervisory experience Responsibilities for MDS Coordinator: Consistently model and demonstrate excellence in customer service, with a constant focus on person-centered care/services delivery. Will schedule the ARD's (Assessment Reference Date) for PPS (Prospective Payment System) assessments with the Rehab Director daily, weekly and as needed in a manner that accurately captures the RUG (Resource Utilization Group) category. Will provide a schedule of ARD's and Assessment types weekly and as needed to the IDT in order to facilitate the timely completion of MDS sections and CAA's (Care Area Assessments) by each discipline. Will schedule all OBRA (Omnibus Budget Reconciliation Act) ARD's daily, monthly and PRN. The schedule will be provided to the IDT. Addendums to the schedule will be provided by the RAI Coordinator to the IDT as needed. Is expected to use the RAI Manual as a resource during the assessment coding process. Will coordinate the completion of MDS sections according to facility assignments. Will communicate to members of the IDT as needed based on timeliness of completion. The RAI Coordinator is expected to report any issues with timely completion to the Administrator immediately. Will facilitate with the IDT members, the completion of assessments and corresponding due dates for Z0400A-L, Z0500B, V0200B2 and V0200C2 according to the RAI Manual. Will ensure the transmission of MDS's to CMA, as per the RAI manual timing guidelines. Will confirm the transmission file and review the printed initial and final validation report. Will facilitate the correction of any fatal errors immediately and retransmit the assessment until an accepted validation report is received. Will also address non-fatal errors using the QIES (Quality Improvement and Evaluation System) ASAP (Assessment Submission and Processing) System MDS 3.0 Provider User's Guide. Will ensure the BO (Business Office) designee receives the initial and final validation report that acknowledges the acceptance of the MDS into the QIES ASAP. Will facilitate the completion and updating of resident care plans within the MDS Department and the IDT to reflect the resident's most current needs. Will identify residents in need of a Significant Change in Status Assessment based on criteria in the RAI Manual. Will coordinate the interdisciplinary care plan team meeting weekly and as needed. Has knowledge of residents' overall care needs and knowledge of where residents reside in the facility. Will participate in facility staff education as it relates to the RAI process and the coordination of obtaining accurate documentation from direct care staff. Will notify the facility DON (Director of Nursing) and/or Administrator of risk areas when coded on the MDS. Will participate in quality improvement activities in the facility. Will compile data reports based on MDS data as requested by the Administrator or DON. Will update information for the CNA (Certified Nursing Assistant) care plan/kardex. Will utilize the facility's software program and computer system to comply with federal requirements for completion and encoding. Participates the in the survey process.
