Lead Case Manager, Rapid Rehousing Program *Newly created position The Night Ministry of Chicago, IL is looking to hire a Lead Case Manager for, Rapid Rehousing Program to work with our Youth Programs team. This Lead Case Manager, Rapid Rehousing Program will promote The Night Ministry's mission to serve and empower young people emerging from homelessness in an innovative program, utilizing best practices including harm reduction and trauma-informed care to gain housing independence. The Lead Case manager for TNM's RRH program is responsible for developing the program documentation for and implementing intensive case management services to residents of The Night Ministry Youth Rapid Rehousing Program, including intake and assessment with new residents, collaborative development of individual service plans with residents, crisis intervention and linkages to resources/referrals. Services are provided both in the office and in the community. The Case Manager Supervisor works to assist residents in goal attainment, including but not limited to: maintaining their housing, obtaining and maintaining sustainable income, and achieving health and wellness goals with the objective of residents being prepared to take on full responsibility for market-based rent and leases within a 24-month period.  It is anticipated that t he Case Manager Supervisor will supervise both a Case manager and a Peer Navigator for TNM's RRH team. If this sounds like the opportunity for you to build a career while fighting homelessness, apply today ABOUT THE NIGHT MINISTRY With open hearts and minds, we provide housing, healthcare, spiritual care, social services, and human connection to members of the Chicago community who are struggling with poverty and homelessness. A DAY IN THE LIFE OF A LEAD CASE MANAGER , RBH As a Lead Case Manager, RBH , lead a cooperative team to implement the goals and service philosophy of the youth programs., and assist in the collaborative development and implementation of programming to meet residents' needs. Supervise and support RRH staff; including recruiting, hiring, training, and performance reviews. Coordinate with The Night Ministry's Youth Outreach Team and Diversion Specialist to identify and enroll youth eligible for the program. The Case Manager Supervisor, RBH will lead and provide case management services including intakes, assessments, service planning, individual counseling, referral, and advocacy for approximately 12 residents at a time. Provide crisis intervention in mental health and/or medical emergencies . QUALIFICATIONS FOR A LEAD CASE MANAGER, RBH Bachelor's degree in Social Work, Counseling, or related field, or significant experience in related work with vulnerable populations 3-4 years of case management experience with at-risk youth 1-3 years of supervisory experience preferred Demonstrated experience working with youth; must be trauma and LGBTQQIA competent. Awareness and endorsement of Harm Reduction philosophy and trauma-informed approach to services Ability to communicate effectively across all programs and partners Must meet all governmental, agency, and grantors' qualifications for the position. Must be able to work independently and in a team Must exhibit independent judgment and sound decision-making ability in a fast-paced work environment. Ability to respond to emergencies quickly, calmly, and in compliance with Night Ministry policies and procedures Team player and team builder - able to work with all personalities and maintain cohesion in a diverse environment Current driver's license, evidence of insurability; access to a vehicle for community-based case management services preferred Must be organized, attentive to deadlines, and able to problem-solve creatively READY TO JOIN IN OUR FIGHT AGAINST HOMELESSNESS?
