Position Summary: Under limited supervision, accesses, plans, and implements services and resources that include approved activities which best meet the needs of the clients, based on the programs goals and objectives. The work is normally reviewed upon completion to ensure compliance with general organization/ department goals, policies and procedures. Major work activities are covered by procedures or guidelines permitting flexibility in selecting methods to utilize and procedures to follow, determination of work priorities, and the planning and coordination of projects. Essential Duties and Responsibilities: ( Depending on area of assignment, incumbent may not be required to perform some of the duties listed below) : Performs initial screening to determine appropriateness and eligibility of services. Conducts intake assessments and orients new clients. Develops case treatment plans. Serves as a client advocate. Arranges client appointments such as but not limited to behavioral, health, medical, dental, educational, financial, family visits, etc. Refers clients to other agencies and resources as needed. Manages case treatment plans for clients to include multiple service plans incorporating referrals and follow-up with other agencies and resources. Maintains communication with other resources, and agencies regarding case treatment plans. Supervises staff in execution of case plans, medical appointment, therapeutic activities, case documentation etc. Provides crisis intervention by responding to client's needs during acute emotional and/or physical stress. Assists in planning, designing, and implementing individual therapeutic community programs. Provides supervision that includes the safety and security of clients, employees and property, in the absence of the Supervisor. Documents timely and accurately the results of the assessment and treatment plans, reports, progress notes, discharge summaries and other resident-related data; generates reports to include all clinical, prevention, and educational activities. Works with therapists in assessing recreational activities for their therapeutic value and applicability to the program objectives. Supervises the coordination of case management training for staff. Exercises discretion and independent judgment to recommend removal of clients into more intensive residential treatment facility, based on evaluation and behavior. Evaluates clients' participation and documents the strengths, weaknesses, and interests of each participant according to established guidelines, policies, and procedures. Reviews program, group, and individual progress to ensure ongoing improvement and participation in the program. Keeps leadership and staff informed of program activities by participating in staff meetings. Coordinates programs with other divisions. Attends weekly or monthly meetings as scheduled to ensure the effective and efficient treatment of cases. Transports residents to/from service agencies appointment or outside activities as needed. Follows up on all cases to ensure appropriate services are provided. Maintains professional and technical knowledge by attending seminars, educational workshops, classes and conferences; reviewing professional publications; establishing networks; participating in professional societies; conferring with representatives of contracting agencies and related organizations. Performs other job related duties as assigned and contributes to a team effort. Knowledge, Skills, and Abilities: Knowledge of Tohono O'odham traditions, language, history and culture. Knowledge of applicable federal, state, tribal laws, regulations and requirements. Knowledge of the economic, educational, and health and social problems of Native Americans. Knowledge in records management procedures. Knowledge of department organization, functions, objectives, policies and procedures. Knowledge of Indian Child Welfare Act (ICWA). Knowledge of case management processes and procedures. Skill in conducting interviews for individuals and groups with empathy and enthusiasm. Skill in operating various word-processing, spreadsheets, and database software programs. Skill in analyzing problems, projecting consequences, identifying solutions, and implementing recommendations. Skill in problem-solving and appropriate decision-making. Skill in providing superior customer service to external and internal customers. Ability to exercise independent judgment. Ability to communicate efficiently and effectively both verbally and in writing. Ability to work effectively with individuals and demonstrate leadership and team-building skills with empathy and enthusiasm. Ability to handle multiple tasks and meet deadlines. Ability to work extended hours and various work schedules. Ability to establish and maintain positive and effective working relationships with other employees and the general public. Ability to maintain privileged confidential information. Ability to travel within the interior/exterior boundaries of the Tohono O'odham Nation. Minimum Qualifications: Bachelor's Degree in Social Work or closely related field and one year work experience in a social services setting or an equivalent combination of training, education, and work experience which demonstrates the ability to perform the duties of this position. Two years of supervisory experience Licenses, Certifications, Special Requirements: Upon recommendation for hire, a criminal background and National FBI check is required to determine suitability for hire, including a 39-month Motor Vehicle Record. May require possessing and maintaining a valid driver's license, (no DUIs or major traffic citations within the last three years). If required, must meet the Tohono O'odham Nation tribal employer's insurance requirements to receive a driver's permit to operate program vehicles. Based on the department's needs, incumbents may be required to demonstrate fluency in both the Tohono O'odham Language and English as a condition of employment.
