CASE MANAGER GENERAL DUTIES & RESPONSIBILITIES Case Manager shall assist individuals and their support networks as they identify, select, obtain, coordinate and use both paid services and natural supports that may be available to enhance the person's independence, integration and productivity consistent with the individual's capabilities and preferences. Complete confidentiality is required. Must be responsive to the needs of the Case Management Program and be willing to be assigned other duties as needed. The Case Manager position is directly responsible to the Case Management Director. Manage assigned individual case records. Assure all necessary documentation is up-to-date and meets and/or exceeds corporate and state standards. Assure all deficiencies noted through Case Records Review and Program Quality Assessment process are corrected immediately for case files assigned. Regularly monitor needs and appropriateness and quality of services provided to individuals. Make home visits to residential programs and be available to person served and family for crisis intervention, making referrals to community agencies as necessary. Maintain and update BASIS information for individuals on assigned caseload. Act as chairperson or coordinate Person Centered Support Plan (PCSP) meetings and other planning meetings as needed, advocating for individuals served in assigned caseload. Complete monthly Targeted Case Management (TCM) billable activities for caseload. Cooperate with staff in employment, educational, habilitation or residential plans to support individuals served in least restrictive environment. Develop and mobilize "natural support networks" such as family, church, and friends in the community. Help develop locally needed services enabling individuals to receive the necessary Home and Community Based Services (HCBS). Establish links between individuals and service providers. Determine financial eligibility in accordance with Kansas Department for Aging and Disability Services (KDADS) regulations. Assist individuals in accessing SSI, Medicaid and other financial benefits. Punctual attendance and completion of assigned shifts; flexibility. Practice responsible management of one's personal time off. ACCOUNTABILITY The Case Manager may be assigned additional duties by the Case Management Director. QUALIFICATIONS Case Manager, preferably, will have a Bachelor's Degree (human services field or a related discipline) and 6 months experience working with individuals with intellectual and developmental disabilities. Experience with individuals who have intellectual or developmental disabilities may be substituted for a Bachelor's Degree at the rate of 6 months of experience for each semester required to obtain the degree. Valid Driver's License and the ability to be covered by corporate vehicle insurance. Must present a professional manner and possess working knowledge of computers. Must provide a means for emergency contact by corporation personnel. Must have the ability to: perform physical maneuvers such as bending, kneeling, stooping, reaching, etc.; recognize natural teaching opportunities and act upon them; be sensitive and caring regarding the personal care needs of persons served; respond to undesirable (sometimes aggressive) behaviors while maintaining calm and composure; be physically and mentally capable of operating corporate vehicles in a safe manner; report for work on site, on time, work for the full shift, and be flexible regarding work location; communicate through verbal, written and electronic mediums; observe, ask questions, listen actively, and give and accept feedback effectively; maintain a high energy level, adaptability, assertiveness, patience and ability to work in team environment. TRAINING REQUIREMENTS Attendance and completion of annual and safety training, Mandt, CPR, First Aid, and Wheelchair Securement. Must meet KDADS licensing requirements. Further educational/certification requirements may be assigned. HAZARDS, RISKS, DISCOMFORTS OF PARTICULAR JOB Case Manager may perform tasks, which on occasion may potentially expose the employee to blood borne pathogens, infectious materials, physical and/or verbal aggression. Employee may be required to perform tasks involving heavy lifting and bending. Employee may be exposed to raw materials and/or chemicals in the work area. May operate motor vehicles and/or equipment including but not limited to computer, fax, photocopier, calculator, typewriter, telephone, paper shredder, wheelchair lift, transfer lift, audio/visual appliances. PERSONAL PROTECTIVE EQUIPMENT Protective gloves, gowns, face masks, shields, goggles, and biohazard waste disposal bags. Tri-Ko will endeavor to provide individual training, protective equipment and/or ergonomic equipment aids, or accommodations as needed to prevent hazards that may relate to tasks required of this position. POSITION CLASSIFICATION/HOURS OF WORK/BENEFITS This position is classified as a non-exempt position. Wages will be paid at an hourly rate. The general hours of work are 8:00 a.m. - 4:00 p.m., Monday through Friday. Hours may vary to include evening, night and/or weekend hours as indicated by the needs of individuals served and the corporation. Full employee benefits are available for this position.
