We recognize the important work that our Navigators - Case Managers do, and provide competitive compensation , with expected pay ranging from $45,000 to $60,000 per year. YOUR ROLE AS A NAVIGATOR - CASE MANAGER As a Navigator - Case Manager, you work one-on-one with our nonprofit program participants to help them develop their own unique Mobility Action Plan (MAP) for their careers and personal lives. With your guidance, participants have access to relevant assessment tools that help them identify their strengths and overcome their challenges. You go beyond standard case management. You're an advocate for each individual, working to address their unique needs and help them achieve their goals. And it's not just about finding employment - you also connect participants with important family support resources and be a driving force behind a two-generational approach to success. You are responsible for coordinating with external providers and ensuring that participants are making meaningful progress toward their goals. You document progress in Electronic Case Management (ECM) systems, assist in compiling data for progress reports, and participate in regional and online learning collaboratives to continue growing your own skills and knowledge. Passionate about social work and helping others, you love coming in to work each day to make a difference! QUALIFICATIONS FOR A NAVIGATOR - CASE MANAGER 3+ years of experience in vocational rehabilitation, case management, or a related field, or work experience determined to be acceptable to the agency Mental health first-aid certification, or the ability to become certified within the first six months of employment A bachelor's degree in social work, social services, counseling, business administration, or a related field from an accredited institution is preferred but not required.
