SUMMARY The Wellness Nurse is responsible for personal care and medication management of the residents in the SVL Assisted Living, including compliance with all aspects of AzDHS State rules and regulations. ESSENTIAL FUNCTIONS In collaboration with the AL Manager/DON develops and implements written policies and procedures required for the day-to-day operation for the medical management of Assisted Living residents: Assures compliance of community policies and procedures by caregiver personnel. Responsible for resident assisted living care: Ensures each resident chart contains the appropriate and required documentation related to the care of the residents. If the resident is incapable of handling his/her financial affairs, notifies the AL Manager and/or the resident s representative. If no resident representative has been designated, contacts a public fiduciary, a trust officer, or other agency to take responsibility for the resident s financial affairs. Ensures each resident is assisted in exercising his-her rights. Notifies physician and family of resident s condition and behavior changes as appropriate and of resident refusal of services (health, medical or assisted living services). Ensure the changes in condition and behavior is document as well as notification of family and Physician. Refusal of services or non-compliant behavior should be reported to the Director of Resident Services/AL Manager. Ensures a residents condition is documented after an accident, incident or injury and reporting it to AL Manager Plans, analyzes and evaluates care giving needs of our residents. Conducts the initial assessment on each resident prior to move-in. Supervises the creation of a service plan for each resident. Updates the service plan as needed. Attends the Service Plan review. Addresses physical, social, spiritual and psychological needs of residents in a manner that assists them to maintain a lifestyle appropriate to them. Identifies, communicates, and addresses any inappropriate residency. Monitors and assures accurate transcription of physician orders into chart and medication record and that orders are followed. Assists with move-in and orientation of new residents and families. Reviews resident charts, treatment book and medication records to assure quality of care within the facility. Supervises ordering of medications from pharmacy according to established policy and procedure. Assists in monitoring diet orders and communication to Dietary Department. Assures proper assembly of resident medical record. Ensures medication assistance, storage, administration and records are accurate and appropriate. Supervisory responsibilities include: Assists AL Manager in the recruitment and selection of employees and ensures compliance with AzDHS Article 8 rules regarding new hires, including: Training and experience qualifications; Freedom from tuberculosis; Fingerprinting; and Reference checking Directly supervises the caregivers for the Assisted Living Department: Ensures each staff member is fulfilling their job responsibilities and not exceeding the duties outlined in Article 8 rules. Complies and carries out supervisory duties as required by AL Manager/DON. Assists AL Manager in conducting evaluations on all caregiving staff. Trains and evaluates caregiver staff in medication and treatment assistance. Provides initial caregiver education and orientation to caregiver job duties. Is responsible for all education and in-service requirements and needs of all Assisted Living staff. Assists AL Manager in staffing of the A.L. Department. Responds to staffing issues in a timely manner. Rotates on-call with AL Manager. OTHER Works with safety committee and maintenance department so that the facility and grounds are safe and free from hazards while taking into account the physical, cognitive and functional condition of the resident and staff. Responds to resident or personnel grievances per policies and refers them to AL Manager/ Resident Services Director when appropriate. Evaluates emergencies and determines emergency measures to be taken regarding resident care. Examines and gives first aid to residents and personnel within the scope of practice. Ensures completion on incident reports and notification of physician and resident representative. Provides phone consultation to caregiver staff in emergency situations when not at the facility. Performs duties as assigned by the AL Manager/DON. Participates in Resident at Risk meetings to review Independent & AL residents to help determine their need for Assisted Living Services or to determine if AL residents already on services need an increase or decrease in level of care. Attends Risk Management Meetings and Team Management Meetings. English is our chosen business language. Therefore, the ability to read, write, speak, and understand English is essential. Other duties as assigned. MINIMUM QUALIFICATIONS To perform this job successfully, an individual must be able to perform each essential function satisfactorily. Current first aid and CPR certification specific to adults. Has a minimum of 1 year experience in providing health-related services, including working in a health care institution or the professional field of nursing. Ability to communicate well with the elderly and other staff. Must possess good clinical skills. Displays leadership skills. Maintains a current nursing license in the State of Arizona. Effective supervisory skills. Excellent interpersonal skills. Ability to communicate well, verbally and in writing. Ability to work flexible hours to meet requirements of the job. Familiar with rules and regulations of DHS for Assisted Living Centers. Knowledge of abuse regulations and related policies and procedures. Knowledge of Resident Rights and Internal Facility Requirements. Knowledge of Policies and procedures of the Assisted Living Center. Possess excellent customer service skills. Hand and eye coordination. Ability to lift 40 lbs. Transporting residents in wheelchairs over long distances. Must be able to ambulate well to cover the distance of the resident apartments.
