Avant Rehabilitation Center is located in Irving, Texas...the heart of DFW. Avante is a premier skilled nursing facility, offering comprehensive skilled nursing and senior rehabilitation programs. Our programs are individualized to meet each patient s needs. A vital member of our nurse management team, our MDS Coordinator will use the Minimum Data Set results to formulate and implement individual care plans for residents. Seeking candidates who have great computer skills, are highly organized, have a positive attitude and good work ethic, are good at thinking outside the box and problem solving, and are motivated to work both individually and as part of a team! Job responsibilities Prepares and electronically transmits timely reports to the C.M.S. database Ensures the accuracy of medical records and coding procedures Completes Resident Assessment Instrument (RAI) and initial interviews with new admissions Vital member of the Interdisciplinary team Develops care plans with short and long term goals for residents Coordinates with therapy to gather therapy minutes; any other essential resident information/assessments Maximizes facility reimbursement within the state and Federal guidelines Schedules the initial, quarterly, and significant change Minimum Data Sets for each resident In conjunction with the Nurse Manager, monitors special needs of residents on psychoactive drugs, on fall prevention program, and turning and toileting schedules to verify that all disciplines are care planned appropriately, all approaches considered and documentation is complete Education Minimum nursing license as an L.V.N in the state of Texas Long term care experience and knowledge of MDS preferred but not required. We may be willing to train a candidate that we believe to be a good fit for this position. This is an administrative position, Monday-Friday 8am-5pm. Salary is negotiable.
