Support Coordinator (Outreach) Based in communities around Newcastle. Permanent full time (37.5 hpw) Based on rota. Pay £11.51 per hour (£22,500 per annum) and great benefits including Health Cash Plan. Driver preferred but not essential. As our Support Coordinator, you’ll be at the heart of supporting our customers who have complex needs; mental health, learning disabilities and autism, to develop their skills to live more independently in the community. You’ll make a big difference each day as you help customers achieve their hopes and aspirations. Amazing we know! Typical day as a Support Coordinator Creating support plans with your customers (and coordinating other support plans for the service). You will empower customers to develop independent living skills they need, to successfully move on (if and when ready). You will offer support so that customers are able to live fulfilled lives in the community with a package of support. Working closely with external agencies so that there is a seamless approach to support. Carrying out risk backssments, support planning, goal setting and regular reviews. Support clients with housing benefit applications and ensuring that our clients are in receipt of the correct benefits. Supporting with any tenancy related issues reporting repairs, contacting utility companies etc. Fancy going home each day knowing that you have helped change our customers lives for the better?
