Title: Physical Therapy ServicesLocation: Charleston,WVClient: The West Virginia, Department of Health Facilities, Lakin HospitalDuration : Long Term Contract JOB DESCRIPTION:SPECIFICATIONS1.PURPOSE AND SCOPE: The West Virginia Office of Shared Administration, Department of Health Facilities, Lakin Hospital is soliciting bids to establish an open-end contract for physical therapy services. QUALIFICATIONS: Vendor, or Vendor?s staff if requirements are inherently limited to individuals rather than corporate entities, shall have the following minimum qualifications:3.1.Registered physical therapist in the State of West Virginia. 4.MANDATORY REQUIREMENTS:4.1Mandatory Contract Services Requirements and Deliverables: Contract Services must meet or exceed the mandatory requirements listed below. 4.1.1 Vendor will provide in-house physical therapy services for the residents of Lakin Hospital. Vendor must include the following services: 4.1.1.1 Vendor must complete evaluation on all residents and all new admissions. 4.1.1.2 Vendor must be available for consultations at the request of the agency. 4.1.1.3 Vendor must document each resident visit and maintain medical and statistical records in accordance with Lakin Hospital policies and procedures. 4.1.1.4 Vendor shall reassess resident upon consultation request and modify treatment plans all in coordination with the attending physician and appropriate treatment team members. 4.1.1.5 Vendor will recommend restorative treatment therapy for residents identified in need. Develop treatment/care plan for those residents on restorative treatment plan on a monthly basis and re-evaluate residents on restorative treatment plan each quarter for modifications of treatment plans all in coordination with the attending physician and appropriate treatment team members. 4.1.1.6 The vendor will include training of Restorative Therapy personnel according to attending physician orders. The physical therapist would ensure personnel are knowledgeable as deemed necessary by facility requirements and residents? needs. 4.1.1.7 The vendor will participate as necessary in appropriate committees and the indirect supervision (instruction and direction of procedures concerning therapy care) of restorative therapy staff. 4.1.1.8 Vendor will recommend and evaluate adaptive equipment and seating devices. 4.1.1.9 Hours of service will Not Exceed 250 hours in a twelve-month period. Vendor will be required to provide the services in accordance with physician ordered physical therapy. 4.1.1.10 Vendor will coordinate schedule with the program service director (the majority of the time worked must be during normal business hours which is Monday Friday from 8:00 a.m. to 4:00 p.m. excluding major holidays New Year?s Day, Memorial Day, July 4th, Labor Day, Thanksgiving Day, the Friday after Thanksgiving and Christmas Day).
