Personal Advisor : Leaving Care Team : Stockport Are you a passionate, dedicated Personal Advisor looking for your next role? 4Recruitment Services are recruiting a Personal Advisor to join a Leaving Care Team based in Stockport. Umbrella Rate: A GBP 17 per hour Duration: 20/03/2024 : 2 months initially Hours: Monday to Friday, 09:00 : 17:00, 37 hrs/week : Must be flexible : Hybrid Personal Advisor role: Full JD Available Direct work with young people aged 16 : 25 years who are leaving care and making the transition to adulthood to ensure they reach their potential Manage a caseload and keep accurate case records of work Complete visits and keep in constant contact with the young people Contribute to Pathway Plan backssments and provide ongoing backssment with young people to inform Pathway Planning Identify packages of support for young people living in the community and supervise and monitor the delivery of tasks in accordance with the pathway plan to meet young persons needs Provide practical advice, assistance and support to the young people Promote the young peoples independence Support with daily skills such as financial skills and budgeting Liaise with a range of agencies including Early Help and Prevention Service, Education and Careers Advice Service, Police, YOS, Health, Housing, Higher/Further Education providers Develop and maintain close relationships with the young people all partner agencies who play their part in supporting the care leavers Attend meetings, reviews and conferences where necessary Essential Requirements UK Driving Licence, access to a vehicle, business insurance Must have experience as a Personal Advisor in a Leaving Care Team Experience/ knowledge of working with care leavers/ young people in care Highly motivated to achieve the best outcome for the young people Excellent written and verbal communication skills You Must Have Right to Live and Work in the UK Must already have an Enhanced Child and Adult DBS registered to the.
