Location: Mountain State Medicine and Rheumatology Position: Nurse PractitionerIf you are a dedicated and compassionate nurse practitioner looking to make a difference in the lives of patients, we encourage you to apply. We are a private practice not affiliated with any of the hospitals. Our mission is to offer complete Primary Care centered around evidence- based practices and principals. Position will be a part time to full time conversion based on patient volume and needs of the practice. Starting at a minimum of 24 hours per week and increasing to at least 37.5 hours based on volume and need as stated. Job Types: Full-time, Permanent Pay: $50.00 - $65.00 per hour Expected hours: 30-37.5 per week Work setting: In-person Outpatient Private practice Experience: Primary care: 1 year (Preferred) Responsibilities: Provide primary care services to patients ages 16-99 Perform comprehensive physical examinations and assessments Diagnose and treat acute and chronic illnesses Prescribe medications and treatments based on patient needs Educate patients on disease prevention and health promotion Collaborate with other healthcare professionals to ensure coordinated and comprehensive care Manage patient care through utilization management and utilization review processes Maintain Strict quality care initiatives as outlined by our contract with our ACO.
